Post-Placement Evaluation

Maiden name of the pre-adoptive mother and the correct spelling of the name of the child and adoptive parent(s) that will appear on the amended birth certificate to be verified by the pre-adoptive parent(s); adoption to be finalized with attorney.

Obtaining these decades-old confidential records was difficult. Of course, all the identifying information in the case of the abandoned-adopted girl had been redacted, in keeping with privacy laws.
